For example, for my 1999 Ford Taurus SE Wagon 3.0 DOHC (217,000):

Drivebelt Tensioner Assembly

Napa P/N NBH 38181
$76.49

Motorcraft P/N F6DZ6B209DA
$53.79

Is the extra cost worth it? If not, any other brands worth considering? I am willing to pay for better part for that peace of mind.

You can't go wrong with Motorcraft. Its all the quality you will need. I can't see how the NAPA would be any better, although NAPA Gold filters are pretty good.

I would say you can't go wrong with either but the Motorcraft is exactly what you need, nothing more.

I would not be considering any other brands unless you can't afford the Motorcraft

Go with Motorcraft. If only the bearing is bad, you can change it for about $5. It's a standard size. If the tensioner itself is seized, you may be able to free it up with penetrating oil and patience.
