This will be the third time I've had to replace the driver side sway bar bushing due to it sliding out? anyone else have that problem or an idea why? the shop that did the first I trust and didn't charge me for a second one but but im not sure about a third time. Any ideas why its doing this?
How about telling us the year and model? I assume it's a Taurus. Also, you help others in the future to help you by putting that info in your User Profile.
I'd suspect the wrong-sized bushing is being used on first thought.
I've never seen a Gen4 front sway bar, so I have no idea what the bushing brackets look like. (I haven't worn out my FIRST Taurus, yet.) How about some photos of both sides' brackets with the bushings? I suspect a deformed bushing bracket on your problem side.

Trust me, most people here can't just picture the front subframe/undercarriage of your particular vehicle. We are fantastic, but not perfect yet.
